Maintaining the highest quality of life with compassionate caring and knowledgeable management.
Elder Care Management is the process of planning and coordinating the care of the elderly to improve their quality of life and to maintain their independence for as long as possible. Senior health care is integrated with the best possible combination of services such as: senior housing, home care for the elderly, senior socialization programs, financial planning and legal assistance for the elderly.
A care plan tailored for each senior's circumstances is prepared after a comprehensive assessment. The care plan is then put into action and modified when necessary based on the Elder Care Manager's monitoring of the effectiveness of the components of the care plan.
An Elder Care Manager is trained to help families deal with the aging process of their elderly parents by:
- Having working knowledge of senior health and the aging process
- Having a sensitivity to family dynamics
- Understanding public and private financial resources
- Advocating for the senior throughout the continuum of care
